Position Summary: As a Licensed Practical Nurse (LPN) at Florry Creative Care Corporation, you will play a crucial role in providing initial assessments for new clients in Polk County. Additionally, you will be responsible for delivering skilled nursing services to some of our clients, ensuring they receive the highest quality care in the comfort of their own homes. Key Responsibilities:

  • Conduct initial assessments for new clients to determine their unique care needs and develop individualized care plans.
  • Administer skilled nursing activities and procedures to clients, in accordance with their care plans and best practices, ensuring the highest standard of care.
  • Perform home visits as necessary to monitor patient progress and provide continuous care.
  • Administer customer service satisfaction surveys on a quarterly basis to gather feedback and improve the patient experience.
  • Promptly address and resolve any grievances or complaints received from patients to ensure their comfort and well-being.
  • Provide guidance and supervision to CNA's and HHA's to ensure the highest quality of care for our patients.
Qualifications:
  • Current and valid LPN license, in good standing.
  • Recent graduates are encouraged to apply; we value the opportunity to nurture and develop new talent.
  • Strong interpersonal skills and a passion for caring for the elderly and disabled.
  • Ability to work independently and as part of a collaborative team.
  • Excellent communication skills and attention to detail.
