Responsibilities

Collaborates with multidisciplinary teams, provides psychotherapy, coordinates patient discharge planning, complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff within the section of Social Work and the Department of Nursing.

The hospice social work position will require a valid driver's license as well as personal vehicle to make home visits; (mileage is reimbursed at the standard IRS level.) Complete comprehensive assessments, develop care plans, make crisis visits and provide grief counseling and bereavement support including co facilitating grief support groups.  

Qualifications

Social worker with current licensure at the clinical level and eligible to practice independently. An MSW degree is also required from an accredited Social Work program. Candidates must have an advanced knowledge of mental illness, personality development, knowledge of the types of treatment, and skills in conducting individual, group, and family therapy. Must have knowledge of adjustment to medical diagnosis’ and chronic disease. Strong communication skills are needed to interact with patients, patient’s family members, multi-disciplinary team, community agencies, and other resources. Must also have psychosocial diagnostic skills and the ability to communicate effectively in both verbal and in written form. Licensure to practice independently at the clinical level as required by applicable state requirements. 

Valid driver's license required for Hospice area.

$36,306 - $54,485 / year; Education, experience and tenure may be considered along with internal equity when job offers are extended.

Schedule Details

Can negotiate days of the week to be fixed or flexible within M-F. Times are 8:00-5:00 7:30-4:30 or 7:00-4:00.

Weekend Schedule

No regularly scheduled weekends. A few events per year may occur on a weekend.
