Your Specific Responsibilities:
Under the supervision of the Operations and Management Consultant manager-SES, the incumbent to this position will develop and implement injury prevention programs, educate the public on various injury risks, and collaborate with community partners to address specific injury issues. As part of the Health Promotion Team, the Health Educator Consultant will improve community health by participating in the planning, implementation, and evaluation of programs and activities designed to encourage healthy behaviors through individual, policy, and environmental change. Objectives will be met by:
•Understanding the Social Determinants of Health underlying Chronic Disease Risk Factors in our community.
• Employing evidence-based practices to reduce or prevent the risk factors that contribute to chronic disease.
• Using community-oriented, participatory models for creating successful community health promotion initiatives.
• Planning and coordinating the use of educational resources to motivate and assist individuals and groups to adopt and maintain healthful practices and lifestyles.
Duties will include but are not limited to the following:
• Participating in the development and coordination of health needs assessments through public health surveys, focus groups, and interviews.
• Analyzing, interpreting, and disseminating data from the Behavioral Risk Factor Surveillance System (BRFFS), Florida Community Health Assessment Resource Tool Set (CHARTS), and other reliable data sources.
• Collaborating with internal and external stakeholders to determine community health needs and developing plans for meeting those needs.
• Conducting health promotion and public health education classes, workshops and trainings for agency personnel, schools, civic groups, businesses, and the general public.
• Preparing and distributing health education materials, including reports, bulletins, displays, and multimedia content.
• Providing public health information in a culturally competent, linguistically appropriate manner for the purpose of improving health outcomes.
• Educating staff and community partners about human trafficking.
• Creating educational materials, including curricula, presentations, and handouts, to promote human trafficking awareness.
• Conducting evaluations to assess the quality and performance of health education programs specific to chronic disease and injury prevention.
Injury Prevention:
• Facilitate opportunities for collaborative injury prevention efforts in the areas of child safety, bicycle safety, drowning prevention, suicide, fall-related injuries among seniors, and others as identified by the community.
• Develop, create, and coordinate community outreach and prevention programs for targeted populations.
• Plan activities based on performance improvement indicators and evaluate effectiveness of plans.
• Develop and disseminate culturally appropriate educational and training materials to reduce the rates of intentional and unintentional injuries.
• Promote and raise the visibility of the DOH-Seminole Injury Prevention and Outreach Program.
• Create on-demand reports that document all interactions, interventions, education, and information provided.
Community Outreach:
• Screen requests for community outreach events using established internal process.
• Work with staff and program managers to meet community requests.
• Work with and coordinate events with appropriate partners to meet community requests.
• Secure resources including staffing, services, and educational information.
• Serve as a focal point to educate the public on the role of the County Health Department.
• Complete and maintain after-event follow-up reports.
Planning and Research:
• Data Management analysis and presentation: Produce reports on chronic diseases and health disparities by analyzing data from various data sources such as FDOH vital statistics (ESSENCE, FL-CHARTS, Merlin); hospitalization, Emergency Department and Ambulatory Surgical Care (ASC) centers data from Agency for Health Care Administration (AHCA); local EMS; US Census; Community Commons; Robert Wood Johnson’s County Health Rankings & Roadmap and other resources.
• Assist in preparing and releasing DOH-Seminole’s Annual Report.
• Develop and implement assessment plans to determine the site’s health and lifestyle needs.
• Work with management to utilize best practices that promote a culture of wellness.
• Oversee program delivery and evaluation, integrating cost-effectiveness with customer satisfaction.
• Primary Data Collection: Develop “Health Assessment” survey tool for data collection in local communities; collect data in targeted communities which will be used to prepare special reports focusing on health disparities in targeted neighborhoods in Seminole County.
Other duties:
• Lead and contribute to writing grants and securing other program funding opportunities.
• Lead and participate in activities to support the Healthy Communities Florida work plan.
• Assist in the achievement and maintenance of Public Health Accreditation status.
• Attend team meetings, in-services, and trainings that support professional development.
• Cross-train to serve as backup to other departments as needed.
• All other duties and responsibilities as directed by the supervisor.
